Healthier Washington Practice Transformation Support Hub

In partnership with Healthier Washington, the Washington State Department of Health (DOH) launched the Practice Transformation Support Hub(Hub). The Hub is a 2.5-year project that will provide WA State practices with the training, coaching, technical assistance, and tools needed to achieve the goals of integrating physical and behavioral health services, moving from volume to value-based services, and improving population health through clinical community linkages. 

The Hub consists of three service areas, each funded by a separate DOH contract:

  1. Regional Connector Network
  2. Practice Coaching, Facilitation and Training Services 
  3. Resource Portal 

The AIMS Center serves as subject matter experts on bi-directional physical and behavioral health integration through a subcontract with Qualis Health for the first two service areas. This support includes training Qualis Health practice coaches in bi-directional integration and population health principles, ongoing guidance to the coaches/connectors, and practice training and technical assistance.
